How Pregnancy Resource Center Tax Credit Works

1. Donate to Mary’s Choices (check, cash, PayPal)
2. Fill out the Pregnancy Resource Act Contributor Application **(K-96 form) and mail It with your check back to

                PO Box 8689
                Topeka, KS 66608

3. You may also contact: mcolbern@maryschoices.com and request the K-96 form be emailed or mailed to you.
4. Mary’s Choices will submit the application to the Kansas Department of Revenue on your behalf.
5. Kansas Department of Revenue will process your application, and you will receive a credit if you are deemed eligible.
